CCS Case Management will provide high quality Case Management services that are responsive to the needs of their clients. CCS Case Management provide a service which includes overseeing, accessing, implementing, and managing personal, therapeutic, and community-based programs for clients with a disability.
CCS specialises in providing case management support to clients with an ABI and spinal cord injury as well as clients with dual disability including drug and alcohol and mental health issues. Our Case Managers are knowledgeable in working with complex clients, across multidisciplinary teams and with Guardians and Administrators.
Complete Care Specialists can assist you with:
- Setting up complex care programs
- Coordinating complex therapy programs to assist you to achieve your goals
- Providing support at times of crisis or where clients or their programs are “at risk”.
- Assisting with discharge planning and transitioning from hospital to home/supported accommodation or nursing home
- Linking clients into community supports, mainstream and disability specific
- Assisting with planning for and implementing NDIS plans
- Completing VCAT applications for Administration / Guardianship
- Assisting clients with navigating government bodies including Centrelink, applying for DSP applications etc
- Assisting clients with housing including DHHS, NDIS and private rental / purchase
- Advocacy
