- Our Clinical Nurse Consultant can provide advice and education to partners, children, family, and friends on how to communicate with and manage your loved one’s dementia / challenging behaviours. We can provide education around medications and behaviours associated with the disease process.
- Providing education and advice around Guardianship, Administration, Powers of Attorney and Supportive Decision Making.
- Completing applications to VCAT for Guardianship / Administration
- Providing short term specialist consultancy for NDIS planning and Aged Care requests.
- Complete applications / referrals for services
- Assist in setting up complex care programs
- Consult with regards to Future Planning
- Provide education around Dementia, Disabilities, Services, funding options and supports.
